Cigna requires pre-authorization for all inpatient behavioral health services, including detox. The authorization process is managed by Evernorth Behavioral Health clinical reviewers.

When you seek treatment, the facility contacts Evernorth with clinical information including substance use history, withdrawal risk assessment, prior treatment history, and current mental and physical health status.

Cigna uses ASAM criteria and other clinical guidelines to determine the appropriate level of care. Decisions are typically made within 24-72 hours, though urgent cases may be expedited.

Concurrent review occurs throughout the treatment stay. A facility clinician will communicate regularly with Cigna to update on clinical status and request authorization extensions when medically necessary.
